Grace Outreach Medical Ministry
Concept Paper 2 September 2009

1.  Purpose: To demonstrate Christ's love and bring people to Him by providing medical services for those in need in the Dalton area and overseas.

2.  Services to be provided

a.  House call visits by family physician and volunteer nurses, allied health providers.

b.  Assistance in non-medical issues that affect health, such as housing rehabilitation, financial counseling, nutrition, transportation.

c.  Assistance in accessing already exiting community resources.

d.  With cooperation of Mission to the World, assist medical personnel traveling overseas as teams to serve the poor.

3.  Benefits to the community

a.  Provide an alternative method of care for patients who do not qualify for any other programs.

b.  Reduce preventable hospitalizations and debilitating illnesses among patients unable to pay for their medical care.

4.  Methodology

a.  Establish a “free clinic” and a 501(c)(3) organization to support it.

b.  Take referrals from local hospitals and social services.

c.  Make house calls the primary vehicle for health care, thus providing a wider range of useful services to a reduced number of individuals and avoiding the costs of operating a fixed facility.

d.  Use high-tech to augment home visits, such as wireless internet enabled electronic medical records.

e.  Seek volunteers such as retired medical personnel and laypersons to augment services.

f.  Form alliances with the medical community of Dalton.  Seek donations and discounts on basic medical services, especially those related to health maintenance.

g.  Form alliances with sympathetic medical specialists to provide higher level care for needy patients.

h. Seek ways to recycle medications and medical equipment for the benefit of needy patients.

5.  FAQ’s:  Frequently Asked Questions

a.  Why provide services by house calls rather than in a clinic?

House calls assist patients who have difficulty traveling to doctors.  They allow more personal service and assessment of needs beyond medical conditions.

b.  Who will be eligible for services?

Persons who have no regular doctor or health care provider and who have no means to gain one.  Enrollment will be limited by the availability of doctors.  Presently, Dr. Smith is the only doctor in the program.

c.  How much will it cost to receive medical care?

Visits by a doctor, nurse or other volunteer will be free.  If it is necessary to prescribe medication or order lab tests from other medical organizations, patients will be responsible for those costs.  Patients will be welcome to make donations to our organization.

d.  When will you start providing services?

Our organization is in the process of filing for incorporation and recognition as a charitable organization.  We hope to start providing services before the end of 2009.

e.  How will Grace Outreach Medical Ministry be supported?

We are applying for recognition as a 501 (c) (3) charitable organization.  Once our application has been received by the IRS, donors may contribute funds to us. These donations will become tax deductible once our organization has been approved by the IRS as a 501 (c) (3) organization.
